A living book.

An evolving collection of memories, experiences, medical history and reflections, documenting a life that has been profound in some ordinary ways, and in some horrific ones.

It is a record of what happened, how it happened, what was known at the time, and what became apparent afterwards, sometimes following seemingly ordinary events.

Some of it is difficult to comprehend.  Much of it raises uncomfortable questions about the systems, decisions and circumstances that shaped what came next.

It is a record of a life as it was lived, and continues to be lived

Whatever meaning we assign to the things that happen to us, to the things that happen with us, and to the things we do ourselves, the consequences can reach much further than we might or could imagine.

Preventing harm to others and ourselves matters because it’s effects can spread through an entire life.

Not all harm is deliberate.
Sometimes it happens through decisions that seem reasonable at the time, or through reasons that were not thought about, assumptions, omissions, failures of communication, unconscious prejudice, or systems that leave important information unseen or unknown. 

But the absence of intent does not necessarily mean the abssence of harm.

Harm that is not recognised can be particularly difficult to address.  If nobody knows what happened, nobody takes responsibility for it, and nobody understands what it has set in motion, the consequences can continue in harmful ways and also affect other lives. 

The black box

I have a large, opaque box in my room containing years of medical records.

A record is evidence that something was recorded. It isn’t automatically evidence that the recorded account is true.

I have to try and save myself. It’s been 16 – 18.5 years of rumination. My nervous system is fried right now. Facing another chronology, trying to put this story down in in order, makes me feel a visceral reaction of some kind. I know why, to a degree, I’ve tried to do this for various reasons while going through the actual physical pain and not succeeded. This time I want it to be completed here.

2007 (late) First child born
2008 (earlier) Visited NHS Dental practice for the first time. Best friend worked there as a Dental Nurse. I believe free NHS dental care was being advertised at the time for women during pregnancy and after having a baby. I went for a routine check up. I had no symptoms. Was told by Dentist I needed further treatment. I did not think about the possibility that Dentists could over treat the general public due to the financial incentive of more treatment meaning more money.
2008 (mid) Returned for the treatment. I felt searing pain during the filling.

Face swelled returned to dental practice. Told didn’t really know what was wrong and then told Gingivistis and to floss. (Later Ombudsman report stated that infection showed on X-ray but I was not told.)

Left eyelid area began to swell intermittently.

Went to GP lump in mouth – GP referred to Maxillofacial

Shutting eye felt like glass

2009 October Saw Maxillofacial Consultant, pretty girl and I’d hate for you to loose the tooth. Said it could be as all connected. Why didnt he offer to extract my tooth? I would have had it extracted there and then due to the whole situation, definitely. He said *** were a bad referral practice and they had had many bad referrals from them. He was angry with them and said that I should not go back. He said go to your previous private dentist. He would have known though, as a Consultant in the NHS that he was creating a potential fragmented pathway for me now, as there was a conflict of interests re the private dentist not wanting to be involved in any bad treatment from a NHS dentist which I had no clue (at age 23) would be an issue.

Antibiotics – triggered skin along with infection and stress from the medical non treatment at the time

Put on medications for the skin condtiions (arriving from the stress – V?)

Pregnancy

Refused childhood Private practice note on file “Not to be seen by * * ****”, put on hold can’t chop and change NHS to private. Despite having been specifically referred from the hospital – I didn’t know that this might have been something I could challenge further. Dentist later replied to my complaint saying he did not know I was pregnant. I don’t know whether the practice staff knew I was pregnant when I contacted them, I would have thought I would have told them. I did not understand at the time that this might be something I could challenge further.

Saw a dentist via work collegue – ยฃ1800

He also faxed hosptiall, chronic absess ? drilled into all my teeth, changed mercury fillings, referred me to endontist – hour long procedure adrenaline etc tipped up while pregnant head light. Why did they not take the tooth out?

Tried to find another dentist – told no by woman. I was crying

A&E
SHO S – magic wand, jaw wrap (crying in Xray reception room as they were saying re pregnancy and am I sure I want to do this), bounced back to dentist – but I didn’t really have one?that was able to tell me what was wrong?
Prev Consultant would have known what to do, I didn’t necc need the jaw wrap if they’d ‘just’ asked Mr F (Maxillofacial Consultant who had seen me October 2009)

Cant remember whether I left A&E with no treatment but a note to come back in 3 days for a follow up (after giving me instructions to find a dentist – having been to many). At some point thoguh Mr S SHO gave me Augmentin Antibiotics despite me being extremely unwell and sick, plus the facial infection, plus the growth slowing regarding my unborn baby. It was at this time I developed severe debilitating skin disease. chronic urticaria, skin writing, vulval, and scalp.

Medicatations for these conditions, given high dose neuropathic medications by someone who has since had their prescribing abilities removed.

Wanted VBAC, but due to growth issues in utero wanted babe out earlier (growth scans). Had I been treated for the facial infection properly – I.e seen the Maxillofacial COnsutlatn at A&E or SHO got advice from him, I potentially wouldn’t have had the subsequent rectal issues (now colostomy)



C-section, felt loss of sensation (spinal? Tugging? plus the neuropathic meds which can cause dyssnergia), on drip for face infection at the time 5 days with newborn

Survived until 2013/14 Loaded, womens health letter never saw

I believe there had been concerns about this going back to the 1990’s, although I had never heard about them, regarding over treating children which was investigated at the time – in September 1997 the government announced a crackdown after cases involving dentists claiming for work not carried out, imaginary patients and overcharging. In 1999, a disciplinary hearing heard allegations that a very highly paid NHS dentist had carried out unnecessary work on children and claimed NHS fees for treatment not provided. The really explicit “dentists doing unnecessary treatment to increase earnings” story became particularly prominent around 2000, when Professor Aubrey Sheiham alleged that large amounts of unnecessary scaling, X-rays and fillngs were being performed and financially incentivised. That led to an Audit Commission inquiry in 2002 into whether dentists were boosting earnings through excessive treatment.

April 2000, The Observer published a major inverstigation following claims by Professor Aubrey Sheiham of UCL that dentists were carrying out substatial amounts of unnecessary treatment and that the payment system created financial incentives to do more work.
